Reconstitution

  1. Use bacteriostatic water (BAC water) for reconstitution of lyophilized vials.
  2. Inject slowly down the inner wall of the vial — do not spray directly onto the powder.
  3. Roll the vial gently between your palms. Do not shake.
  4. Refrigerate immediately. Use within 28 days.
Example: 5 mg ÷ 1.0 mL BAC water = 5 mg/mL

Dosing Protocol

1Initiation
Week 1–4

2.5 mg weekly. Allows GI system adaptation and baseline tolerance assessment.

2.5 mg / week
2Titration
Week 5–20

Increase by 2.5 mg every 4 weeks: 5 mg (wk 5–8), 7.5 mg (wk 9–12), 10 mg (wk 13–16), 12.5 mg (wk 17–20, optional step).

5–12.5 mg / week
3Maintenance
Week 20+

15 mg weekly maintenance. Some protocols may go up to 30 mg under close medical supervision.

15 mg / week (max 30 mg)

Inject once weekly, same day each week. If a dose is missed and the next scheduled dose is more than 3 days away, administer the missed dose. Otherwise, skip and resume on the next scheduled day.

Administration

1
Prepare
Wash hands thoroughly. Gather reconstituted vial, 1 mL syringe, alcohol swabs, and sharps container.
2
Swab
Wipe the vial stopper with an alcohol swab. Clean the injection site (abdomen, thigh, or upper arm).
3
Draw
Invert the vial and withdraw the prescribed dose using the dosing table above.
4
Inject
Pinch skin fold at the site. Insert needle at 45–90°. Inject slowly and steadily.
5
Rotate
Alternate injection sites weekly. Avoid the same spot within 7 days to reduce irritation.
6
Log
Record date, dose, injection site, and any side effects. Track weekly weight for progress monitoring.

Storage & Handling

Storage
  • Lyophilized: 2–8°C (refrigerator), protect from light
  • Reconstituted: 2–8°C, use within 28 days
  • Caution: Never freeze reconstituted solution
Solution Checks
  • Solution should be clear, colorless to slightly yellow
  • Discard if particulates, cloudiness, or discoloration appear
  • Label the vial with reconstitution date

Side Effect Management

Most side effects are gastrointestinal and dose-dependent. They typically improve within 2–3 weeks at each dose level. The gradual titration protocol above is designed to minimize GI impact. Report persistent or severe symptoms to your healthcare provider.

Nausea (Most Common)
  • Eat smaller, more frequent meals
  • Avoid fatty or greasy foods
  • Ginger tea or ginger supplements may help
Reduced Appetite
  • Expected therapeutic effect
  • Ensure minimum nutrition intake (protein priority)
  • Stay hydrated — aim for 2–3 L water daily
Injection Site Reactions
  • Rotate injection sites each week
  • Apply ice before injection to reduce discomfort
  • Allow solution to reach room temperature before use
GI Discomfort
  • Diarrhea and constipation may alternate initially
  • Usually resolves within 2–3 weeks per dose level
  • Consult provider if symptoms are severe or prolonged
