## Where Should You Actually Inject Semaglutide?
One of the first questions patients ask us after picking up their prescription: "Where do I put this?" It is a fair question, and the answer matters more than most people realize. Injecting in the wrong spot -- or worse, injecting in the same spot week after week -- can lead to lumps under the skin called lipohypertrophy, which actually interferes with how well the medication absorbs.

## The Three Approved Injection Sites
### 1. Abdomen -- The Go-To for Most Patients
The abdomen is by far the most popular site, and for good reason. Most people have adequate subcutaneous fat here, it is easy to see and reach, and absorption tends to be consistent.
**Where exactly:** At least 2 inches from your navel, avoiding the waistband area. The lower abdomen or sides work well. Think of dividing your belly into four quadrants and rotating through them.
**Practical tip from our clinic:** If you wear a belt or tight waistband, inject well below that line. Friction from clothing can irritate a fresh injection site and cause redness that patients sometimes mistake for an allergic reaction.
### 2. Front of Thighs -- A Solid Alternative
The middle third of the thigh, front or outer side, offers plenty of surface area for rotation.
**Best approach:** Sit down comfortably, relax the leg completely, and choose the fleshy part of the thigh. Avoid the inner thigh (too sensitive) and the area directly over the kneecap.
**Who prefers this site:** Patients who carry less abdominal fat, or those who find abdominal injections uncomfortable. We also recommend thighs for patients recovering from abdominal surgery.
### 3. Back of Upper Arms -- The Assisted Option
The outer, back area of the arm between the shoulder and elbow is approved, but there is a practical catch: most people cannot reach this area well enough to inject themselves.
**Our recommendation:** Only use this site if someone else can administer the injection for you. The fat pad on the back of the arm is often thinner, making proper subcutaneous depth harder to achieve solo.
## Why Site Rotation Is Not Optional
A study published in Diabetes Research and Clinical Practice (2019) found that lipohypertrophy -- hard or spongy lumps under the skin -- occurs in up to 50% of patients who inject in the same spot repeatedly. These lumps are not just cosmetic. They alter medication absorption, making your dose less predictable.
### A Simple Rotation Schedule

## Step-by-Step Injection Technique
### Before You Start
1. Wash hands with soap and water
2. Let the medication sit at room temperature for 15-30 minutes (cold medication stings more)

4. Gather an alcohol swab and sharps container
### The Injection
1. **Clean** the site with an alcohol swab in a circular motion, then let it air dry for 30 seconds
2. **Pinch** about 1-2 inches of skin between your thumb and forefinger
3. **Insert** the needle quickly at a 90-degree angle (45 degrees if you have very little body fat)
4. **Inject** slowly and steadily
5. **Hold** for a full 10-second count before removing
6. **Remove** the needle straight out and apply gentle pressure -- do not rub
### After the Injection
- Dispose of the needle in a sharps container immediately
- Note the date, site, and any reactions in your log
- A tiny drop of blood or slight redness is normal
## Troubleshooting Common Problems
### Bruising
Happens when you nick a small blood vessel. It looks alarming but is harmless. Apply gentle pressure for 30 seconds after removing the needle. If bruising is frequent, try inserting the needle more quickly -- hesitation increases the chance of hitting a vessel.
### Lumps Under the Skin
A sign of lipohypertrophy from inadequate rotation. Stop injecting in the affected area and let it heal (this can take weeks to months). Strictly follow a rotation schedule going forward.
### The Injection Stings
Cold medication is the most common cause. Let your pen sit out of the refrigerator for 15-30 minutes before injecting. Also check that the alcohol has fully dried before inserting the needle -- wet alcohol on broken skin hurts.
### Medication Leaks Out After Injection
You probably did not hold the needle in long enough. Count a full 10 seconds before removing. Some patients count to 15 to be safe. Also make sure you are inserting deep enough to reach subcutaneous tissue. ## As You Lose Weight
This is something most guides overlook. As you lose body fat, your injection sites change. Patients who started with easy abdominal injections sometimes find they have much less fat there after 6 months of treatment. The pinch technique becomes more important, and some patients switch to a 45-degree angle or move to the thigh as their primary site.
